2008 USMS registration
Instructions: click on the name of your club or chapter and the registration form will appear, then Type it--Print it--Sign it--Mail it (these forms are interactive, but they are NOT electronic submission forms -- you will have to print it and postal mail it).

About those club initials: Due to a great deal of confusion among swimmers and meet directors about how to enter and report meet results involving the Florida Aquatic Combined Team (FACT), initials of FACT chapters must be modified by replacing the last letter of the club initials with "F."
For example, Masters who are members of just Space Coast Masters (in the "independent" column below) will use the initials SPCO, while Masters who are members of the Space Coast Masters chapter of FACT (in the "FACT" column below) will use the initials SPCF. All are still affiliated with Space Coast Masters, and all registrations are mailed to the Space Coast Masters registrar. Where there isn't an option, the club has decided that all of its members will be registered with the same group. Swimmers who want to join FACT and whose club does not have a separate FACT registration form should use the Florida Aquatic Combined Team form and print the first three letters of the club's initials in the appropriate blanks for chapter initials.
